Avenues is a program provided by Family Services Windsor Essex.\u00a0 It is a service for individuals with intellectual disabilities who have individualized funding or Passport funds.\u00a0 Staff from Family Services, help families to recruit Direct Support Providers.\u00a0 Families may access the recruitment tool called www.supportyourway.ca in partnership with Family Respite Services in this program.\u00a0 There are fees for this program.<\/p>\n

Family Respite Services Windsor-Essex<\/strong>
\nFamily Respite Services provides in home and out of home respite\/short break services for families of children up to the age of 18. \u00a0Also assists families to recruit support or respite providers through an on line service called respiteservices.com.<\/a><\/p>\n

Family Services Windsor-Essex<\/strong><\/p>\n

Provides a variety of supports, including counselling, and the Adult Protective Service Workers (APSW) program. \u00a0The APSW program provides adults with disabilities assistance with advocacy, planning and personal support, trusteeship and community development and education. Family Services Windsor Essex also provides the “Avenues” program<\/strong> which assists people with developmental disabilities who have Passport funds or individualized funding to recruit Direct Support Providers.<\/p>\n

Working in partnership with St. Clair College paralegal program, Family Services Windsor-Essex offers free assistance in completing government forms and more. \u00a0This service is available every Tuesday and Thursday at the Windsor location from 9a.m.-2:30 p.m.<\/p>\n

Parents of Technologically Dependent Kids of Ontario: Kids Country Club<\/strong>
\nThe Kids Country Club is a wheelchair accessible respite home in London. This program provides a weekend away from home with social and recreational events for eligible children from birth to 18 years of age, who are technologically dependent and\/or medically fragile. The physical and medical needs of all participants are met by professional staff, including Registered Nurses, Registered Practical Nurses, and Developmental Service Workers.<\/p>\n

Partners for Planning (p4p) Planning Network<\/strong><\/p>\n

P4P has a wealth of free planning resources to help people with disabilities live their best lives.\u00a0 The site offers the opportunity to on line discussions and seminars about planning, finances, relationships, staying resilient and more.\u00a0 There is also information about professional services, planning for the future, work roles and contribution, relationships and community, federal and provincial supports, legal and financial planning and creative options for\u00a0home.<\/p>\n

The “Special Needs” Planning Group<\/strong><\/p>\n

The “Special Needs” Planning group is an organization that is made up entirely of parents of people with disabilities.\u00a0 They have a website that includes resources helpful to parents for planning and links to resources on tax and estate planning and other useful information.<\/p>\n

Special Services At Home (SSAH)<\/strong><\/p>\n

The Ministry of Community and Social Services provides funding for children with physical and developmental disabilities up to the age of 18 through this program. Family Respite Services administers the program in Windsor Essex. Family Respite Services, Community Living Essex or Community Living Windsor assist families with managing the funds or families can do this themselves.<\/p>\n

Windsor Essex Brokerage for Personal Supports<\/strong><\/p>\n

Provides individuals the opportunity for unencumbered planning. \u00a0Through facilitation, support is provided to look at dreams, the types of supports and services needed and assistance to develop a plan to access the services. \u00a0The Brokerage service includes opportunities to review options for the use of funding and assist with formal and informal arrangements of contracting services with community service providers.<\/p>\n
